99453– Initial device setup and education

99454– Monthly data transmission and device supply

99457– First 20 minutes of monitoring and interactive communication

Patients with chronic or acute conditions who benefit from ongoing monitoring, such as hypertension, diabetes, COPD, heart failure, or obesity.

Devices must be FDA-defined medical devices capable of automatic data transmission (e.g., Bluetooth-enabled BP monitors, glucose meters).

While manual entry is allowed, devices that automatically transmit data are preferred and often required for full RPM reimbursement compliance.

Vive Health supports integration into your practice through provider dashboards, alerts, and optional clinical support to help with documentation and follow-up.

Yes. RPM is suitable for both chronic care and post-discharge or post-surgical monitoring

RPM services can be delivered by physicians, NPs, PAs, and clinical staff under general supervision. Non-clinical staff cannot independently bill for RPM.

To bill 99457, providers must spend at least 20 minutes per calendar month reviewing data and interacting with the patient. For 99458, each additional 20 minutes is billable.

RPM involves physiologic data from FDA-cleared devices (e.g., BP, glucose monitors).

RTM (Remote Therapeutic Monitoring) captures non-physiologic data like pain scores or exercise adherence, typically used in PT/OT settings.

 No. RPM complements your in-person care by providing ongoing updates between appointments.

The Vive Health devices are designed to be simple and user-friendly. Our dedicated team is here to support you with setup and training.  

RPM is covered by Medicare and many insurance plans. You may have a small co-pay, which is likely close to that of an office visit co-pay.

Your care team will be notified and may contact you to check in, adjust your care plan, or schedule an appointment.

Some devices require Wi-Fi or Bluetooth to send data, but Vive Health offers cellular-enabled options that work without home internet.

Yes. All data is securely transmitted and stored in compliance with HIPAA regulations.

Your care team may send a reminder. It’s okay to miss a day occasionally, but consistent use helps ensure accurate monitoring.

No. Your readings are reviewed regularly, and you will only be contacted if there is a concern or clinical need.

Yes. You can opt out at any time by contacting your provider or the Vive Health support team.
